Additional forms for residents
For Mauritania, the following additional forms are required:

  • Supplementary Information – Your Travels (IMM 5562) (digital form)
      • Must be completed by the principal applicant and all family members aged 18 or older included in the application.
  • Details of Military Service (IMM 5546)
    • Required for anyone included in the application who has ever:
      • held a high-ranking or senior government position, or
      • served in the military, army, defence, police unit, or national service.

A copy of all pages of the military card or booklet must be submitted, including blank pages, if applicable.

Birth certificates
If a child’s birth certificate was not issued at the time of birth, IRCC requires additional supporting documents, which may include:

  • the supplementary judgment,
  • another official document naming the parents,
  • school documents listing the parents (or a written explanation if unavailable), and
  • the household registry.

These documents must clearly establish parentage and be consistent with the information declared in the application forms.

Documents issued in Arabic or any language other than English or French must include:

  • a copy of the original document, and
  • a certified translation that meets IRCC requirements.

Consistency of names, dates, and identity details across all documents is especially important for Mauritania-based applications.
