Additional forms for residents
For Mauritania, the following additional forms are required:

Supplementary Information – Your Travels (IMM 5562) (digital form)

- - Must be completed by the principal applicant and all family members aged 18 or older included in the application.

Details of Military Service (IMM 5546)

- Required for anyone included in the application who has ever:
  - held a high-ranking or senior government position, or
  - served in the military, army, defence, police unit, or national service.

A copy of all pages of the military card or booklet must be submitted, including blank pages, if applicable.

Document Instructions

Birth certificates
If a child’s birth certificate was not issued at the time of birth, IRCC requires additional supporting documents, which may include:

the supplementary judgment,
another official document naming the parents,
school documents listing the parents (or a written explanation if unavailable), and
the household registry.

These documents must clearly establish parentage and be consistent with the information declared in the application forms.
