Country-Specific Documents
- Travel History (IMM 5562): We strongly recommend completing this form for the principal applicant and all family members aged 18+.
Required Civil Documents
Italian municipalities (Comune) issue civil documents in various formats.
- Pro Tip: Always request your Birth (Estratto di Nascita) and Marriage (Estratto di Matrimonio) certificates in the “Multilingual Format” (Plurilingue / Modello Internazionale).
- Why? These specific versions include English/French labels, meaning you do NOT need to pay for certified translations.
- Family Status (Stato di Famiglia): If you are applying as Common-Law, this document is your “golden ticket.” It officially proves who lives in your household and is excellent proof of cohabitation.
Police Certificates (PCC)
Unlike some countries with a single check, applicants aged 18+ who have lived in Italy must generally provide two documents:
- Certificato Generale del Casellario Giudiziale: The General Criminal Record certificate.
- Certificato dei Carichi Pendenti: The Certificate of Pending Charges (issued by the Procura della Repubblica where you reside).
- Note: Both are required to give a complete picture of your background.
