IMM 5562 – Travel History (mandatory for all applicants 18+)

Israeli applicants—and any accompanying family member aged 18 or older—must complete the Supplementary Information: Your Travels form.
This is a strict requirement for residents of Israel, even though IRCC does not require it for every country.

IMM 5546 – Details of Military Service (if applicable)

Because Israel has mandatory military service, many applicants will be required to complete this form.
You must complete IMM 5546 if you or any accompanying family member has ever:

  • served in the IDF (mandatory or reserve)
  • held a high-ranking or senior official government position
  • served in police, defence, or intelligence units

You must also submit a copy of all pages of the military card or booklet—including blank pages.

  • Israel has no additional IRCC instructions for civil status documents. This means:

    • Standard Israeli civil documents are accepted
    • No special authentication or extra government stamps are required
    • All documents not in English or French must be submitted with a certified translation
  • Most Israeli applicants complete biometrics at the Visa Application Centre (VAC) in Tel Aviv) after receiving IRCC’s instruction letter.
