While Ghana does not require unique local forms, two IRCC forms apply when relevant:

✓ IMM 5562 — Travel History (digital)

Required for all applicants and dependants 18+.

✓ IMM 5546 — Military Service Form

Only required if the applicant has served in the military, police, security forces, or held a senior government role.
Must include all pages of the military booklet — even blank ones.

Translations must be done by a certified translator, with the original document included.

  • In Ghana, many children receive their birth certificates years after birth, which IRCC considers a “delayed registration.”

    You must provide:
