Country-Specific Documents

While Ghana does not require unique local forms, two IRCC forms apply when relevant:

✓ IMM 5562 — Travel History (digital)

Required for all applicants and dependants 18+.

✓ IMM 5546 — Military Service Form

Only required if the applicant has served in the military, police, security forces, or held a senior government role.
Must include all pages of the military booklet — even blank ones.

Translations must be done by a certified translator, with the original document included.

Other “Must-have” Documents

In Ghana, many children receive their birth certificates years after birth, which IRCC considers a “delayed registration.”
If this is your situation, IRCC requires multiple supporting documents, not just the certificate itself.
You must provide:

If issued long after birth, it must be supported by the following:

A court-issued confirmation of parentage.
✓ Additional Parentage Evidence
IRCC allows any of the following:
- School records listing the parents
- Household registry entries
- Another government-issued document naming the parents
If any item is unavailable, provide a clear written explanation, or IRCC may delay or refuse the application.
