Country-Specific Documents
IRCC does not require any additional country-specific forms for residents of the Czech Republic. In some cases, applicants may be asked to complete Supplementary Information – Your Travels (IMM 5562) if IRCC needs clarification on travel or residence history.
Other “Must-have” Documents
There are currently no special document instructions issued by IRCC for the Czech Republic. Standard civil documents—such as birth certificates, marriage certificates, or divorce records—must be clear, complete, and match the information declared in the application. Biometrics
